The potential to improve a patient’s quality of life by reducing pain and enhancing function, with minimal adverse effects, has generated interest among researchers, clinicians, and the biotechnology industry in developing new applications for cell therapies.

Adipose tissue has proven to be safe and potentially effective in the treatment of orthopedic conditions. Microfragmented adipose tissue is a relatively new treatment concept, which we have been performing at our Institute, and which meets FDA (Food and Drug Administration) regulatory criteria. It is considered a minimally invasive procedure for extracting and injecting adipose tissue for orthopedic conditions in a single day and time, and is not considered a biological drug like Mesenchymal Stem Cells expanded in an Advanced Therapies laboratory (which do require regulation and controls).

What does MICRONIZATION OF ADIPOSE TISSUE involve?

The procedure involves taking a sample of adipose tissue using a technique called MELA (Mini Ambulatory Lipid Extraction), performed under local anesthesia in 15 minutes. The resulting adipose tissue (50 ml) is then filtered and processed using advanced technology and finally reinjected into the knee joint being treated. The entire procedure is completed within 45 minutes, and the patient is discharged 50 minutes later.

The literature supports this with several studies that have shown promising results.

In 2014, Striano et al. demonstrated, in a single case report, improvements in VAS pain scores and the Knee Injury and Osteoarthritis (KOOS) score 6 months after injecting microfragmented autologous adipose tissue into a severely arthritic knee with concurrent meniscal disease. These encouraging results pave the way for larger studies in patients with knee pain who do not respond to other current treatments.

A recent study by Malanga et al. demonstrated the safety and efficacy of percutaneous injection of micronized adipose tissue in 17 patients with a history of knee osteoarthritis (Kellgren-Lawrence grades 3 and 4). Significant improvements in pain and functional outcome measures were observed at 12 months compared to baseline. Improvements were noted in the Numerical Rating Scale for Pain (NPRS), the Knee Society Score (KSS), and the Lower Extremity Activity Scale (LEAS). No serious adverse effects were reported.

Recent literature has demonstrated the safety of Micronized Adipose Tissue.

Russo et al. evaluated the safety and 1-year outcome of a single intra-articular injection of autologous, micro-fragmented adipose tissue in 30 patients with diffuse degenerative chondral lesions. They found no major complications, either at the knee or at the harvest site. Hudetz et al. demonstrated positive structural and biochemical changes in cartilage after intra-articular injection of autologous micronized adipose tissue in patients with knee osteoarthritis. A total of 17 patients were enrolled in the study, and 32 knees with OA were evaluated. They found a decrease in mean visual analog scale (VAS) scores and an increase in glycosaminoglycan (GAG) content in hyaline cartilage.18 Cattaneo et al. retrospectively analyzed the safety and potential benefits of using autologous, micro-fragmented adipose tissue as an adjunct to the surgical treatment of degenerative chondropathy of the knee. The results demonstrated that, when combined with a shaving procedure, it improves symptoms and function for at least one year of follow-up, with a consistent upward trend over time. In fact, a consistent and statistically significant improvement was observed in all clinical scores from the preoperative assessment to the 1, 3, 6, and 12-month follow-up, with the KOOS sport score and quality of life showing the most significant improvements.

It is an innovative and regenerative treatment for the articular cartilage of the knee (due to degenerative osteoarthritis).
