This is discrediting the field of regenerative medicine and cell therapy, with poorly performed procedures and doctors who have just taken a weekend course and call themselves stem cell specialists.

So how can you make sure you get something real and not fake?

If you want to ensure you are not scammed by those who are not legitimate within institutions, you should answer these simple questions:

1. What are they going to inject?

2. How and where were the cells separated? Was it done in a simple office with laminar flow, or in a clinic authorized for medical practices following Biosafety and preparation standards?

3. Who will perform the procedure?

4. When does it work and when does it not work?

5. In what environment will the injection be administered? Will it be done in a doctor’s office or a sterile room specifically designed for this procedure?

6. How will you carry out the procedure?

7. What is the cell or platelet count applied?

1. What are they going to inject? What are regenerative or orthobiological products?

These substances can be autologous (from the same patient) or allogeneic (from another patient) and are injected to accelerate healing or promote the regeneration of orthopedic tissues. This includes derivatives such as platelet-rich plasma and stem cells, primarily.

2. How and where did they separate the cells?

Is it performed in a simple office with laminar flow, or in a clinic authorized for medical practices following Biosafety and preparation standards?

A serious mistake made by those who are not properly trained in Regenerative Medicine is the separation of biological products to be applied to a joint or soft tissue, for example, within a medical office.

Besides being sites generally not authorized by the Colleges of Biochemists, as required by law, for obtaining and processing products, these are physical locations that do not meet biosafety standards. That is precisely why they are called “Consulting Rooms,” for conducting medical consultations, not for performing applications or procedures.

Many professionals demonstrate that perhaps because they have inexpensive technologies, such as physical biosecurity barriers like sterile gloves or laminar flow cabinets, they have this problem solved.

Unfortunately, this is the case. To properly apply Regenerative Medicine, the collection and separation of Cells or Platelet Concentrates must be done in places where biosafety is guaranteed, such as rooms prepared for extraction and specific processing areas, like a Laboratory, with air showers, positive pressure air in the room, laminar flow biological safety cabinets, and ultraviolet light for room cooling before and after processing.

The only way to do this in one place is by using a closed processing system, where the product will NEVER have contact with the outside.

3. Who will perform the procedure?

The procedure must be performed by a physician trained in tissue regeneration. The physician must not only have knowledge of anatomy and be accustomed to administering injections, but also understand the crucial moment for applying the regenerative product to trigger the desired responses. Understanding these mechanisms involves grasping the essential molecular basis for the regeneration of a joint structure. Furthermore, the physician must understand why the product is being applied and what the response to the biological product will be week after week, following the phases of inflammation, proliferation, and regeneration.

4. When does it work and when does it not work?

The most important aspect of medicine is knowing when to use which treatment and when not to. This last point is critical and important, as not everyone is a candidate for regenerative medicine. It has contraindications that must be properly understood.

Applying an autologous regenerative product is not a guarantee that it will ALWAYS work. To determine the correct procedure, the patient’s data must be properly analyzed and understood.

Some pathologies respond better than others, and it largely depends on the stage of the disease. A thorough understanding of these factors, the pathophysiology, and the regenerative product’s capacity to determine its potential effectiveness is essential. For example, our follow-up data analysis showed that in elderly patients with grade III-IV knee osteoarthritis, our same-day mesenchymal stem cell procedure helped prevent knee replacement and improved quality of life by 67%.

5. In what environment will the injection be administered? A doctor’s office or a sterile room specifically designed for this procedure?

It is very important that when performing a Regenerative Medicine procedure, the physician administering this treatment knows that it must be carried out in specific, specially equipped rooms and not in a regular office for scheduled patient appointments. The biosafety of these therapies must be guaranteed, complying with international requirements and including precautions down to the physician’s clothing.

There are many professionals currently applying cell therapies without masks, with non-sterile nitrile gloves, without caps and gowns, whether these products contain cells or platelet-rich plasma, without correctly identifying the damage they can cause when applied in a medical care office.

Many products are processed and separated in the office, with or without a laminar flow hood, under the mistaken belief that environmental biosafety is guaranteed, when this is not the case. Positive pressure in the room is crucial to prevent air contamination and ensure filtered air renewal.

Regenerative products must be applied in sterile rooms or rooms specifically designed for these procedures. The processing of these products must be carried out in environments or areas specifically authorized by regulatory bodies, and in compliance with recommended international standards. Therefore, it is generally recommended that they be applied in a clinic, where a physician can also address any potential complications. Many physicians use methods to separate these products in sealed containers, thus ensuring the biosafety of the equipment, but clearly these products must be placed in sterile environments.

6. How will you carry out the procedure?

It is important that the medical professional explain the entire procedure to you, from obtaining and extracting the product from the source to its application. It is also important to know if the procedure is performed under anesthesia or if the cell product already contains anesthetic. We frequently see videos circulating on social media showing regenerative products being administered with anesthetic in the same syringe, where cellular cytotoxicity is closely linked to the regenerative products.

7. What is the cell or platelet count applied?

You must be very clear that a doctor who applies a regenerative product and does not provide you with the cell count of the product, whether it be mesenchymal cells, total nucleated cells from the bone marrow, cells from the stromal fraction of fat or even platelet-rich plasma, has clearly performed a therapy without knowing what he has put in.

The importance of cell counting is key and is what allows us to know the therapeutic orientation that the doctor and the patient will have.

Cellular accounting can be done very easily by trained personnel; the problem is that many people don’t know how to do it because they don’t get trained.

In turn, not only is cell counting recommended, but testing of the products is also recommended to determine if they are products with live cells, if they may have been contaminated, and this can be done with very simple techniques.

In conclusion… we are beginning to understand why in Argentina and also in Latin America, good practices in Regenerative Medicine are important and key to its proper development and to prevent therapies from being categorized as fraudulent or miraculous.

In turn, specialists in Stem Cells, or Cell Therapy or Regenerative Medicine, do not exist and are not legally endorsed.

If there are professionals linked to Molecular Biology, Cell Physiology, Hemotherapists or Specialists in Rehabilitation or Traumatology, they would be the ones who could be closely related to these regenerative therapies.

Those physicians who practice Regenerative Medicine and have verifiable academic or scientific endorsement may be called “Experts in Regenerative Medicine and Cell Therapy.”

Written by Dr. Matias Fernández Viña – MP 64.620 Medical Director Stem Cell Therapy Argentina It often happens that we talk to acquaintances who tell us that Regenerative Medicine is a lie, or it doesn’t work, because a friend of an acquaintance underwent a treatment that didn’t work for them.
